Explanation of Benefits (EOB): This is NOT a bill. It’s a statement sent by your health insurance company after they have processed a claim submitted by us or another provider.

  • What it shows: It details what services were billed, what your insurance plan covered, any discounts applied, and importantly, what portion might be your responsibility (deductible, copay, coinsurance).
  • What to do with it: Review it carefully. Compare the services listed with the services you received. Keep it for your records. It helps you understand how your benefits were applied. If you see discrepancies compared to a bill you later receive, the EOB is a helpful reference.
  • Who to Contact for EOB questions: Please contact your health insurance company directly using the customer service number typically found on your insurance card or the EOB itself. They can best explain your specific benefits and plan details.

  • Pathology Services (e.g., Cirrus DX or Hospital Pathology Groups): Analyze tissue samples (biopsies) taken during procedures.
  • Anesthesia Providers: If you have surgery requiring anesthesia, the anesthesiologist or their group will bill separately for their professional services.
  • Surgery Center/Hospital Facility Fees:  If your procedure takes place at an outpatient surgery center or hospital, they will send a separate bill for the use of their facility, equipment, and staff.
  • What this means for you: You may receive separate bills from these partners in addition to the bill you receive from IWI.
